We thank you for partnering with Klamath Hospice and Palliative Care and look forward to working with you in providing the best possible care for your terminally ill patient and their family. Klamath Hospice and Palliative Care is here to provide extra support for your patient and their family.
As your patient’s attending physician you will be asked to:

– Review your patient’s hospice plan of care.
– Review and/or amend the hospice standing orders.
– Receive updates on your patient’s plan of care.
– Participate in telephone consultations with Klamath Hospice and Palliative Care staff.
Your patient and family will have a team of caring and compassionate professionals to care for them.
